Two-minute delirium screens for night shift

I keep a laminated 4x6 card with 4AT on one side and bCAM on the other at the meds cart, and it’s helped our team catch hypoactive delirium early so we can loop in the hospitalist, pharmacy, and family before morning rounds. I stick a note on the vitals rover so every 2 a.m. check includes RASS plus ‘What day is it and where are you?’, and if they wobble I do a 60‑sec Nu‑DESC before paging — fast, but it overcalls when pain or hypoxia’s off so I recheck after meds/oxygen.
