Rapid checklist for high-alert drips

I made a 4x6 pocket card with titration limits, independent double-check steps, and line-label cues for norepi and insulin during the 1900 med pass — takes about 60 seconds and helped us catch two near-misses last month. Happy to share the wording or a blank template if useful — anyone else?

Love the 1900 med-pass focus; add a QR to unit policy and ISMP list: https://www.ismp.org/recommendations/high-alert-medications-acute-list. Just watch for policy revisions.

That pocket card sounds super helpful! I would love a blank template if you can share. Have you tried incorporating any color coding for easier access?
