Navigating multidisciplinary care for elderly patients

I’ve been reflecting on how critical a team approach is when caring for our elderly patients, especially those with chronic conditions. Using tools like care coordination platforms can really help streamline our efforts and ensure everyone is on the same page. It can feel like herding cats when coordinating care, but I’ve found that daily huddles really help keep everyone aligned. Just yesterday, a quick chat with the pharmacist caught a med interaction that could’ve been missed otherwise. @Teamwork in Nursing definitely makes a difference.

I totally agree on the importance of teamwork — in my experience, involving social workers early on has been a game changer, especially for our elderly patients. They can really help bridge gaps that healthcare providers might overlook.
