CMS finalizes nursing home staffing minimums

A couple of you flagged the CMS rule last week — appreciate the heads-up. It sets 3.48 HPRD with at least 0.55 RN and 2.45 NA, plus 24/7 RN coverage. Our partner SNF is already advertising for five RN FTEs; how are your facilities planning to staff up during the 2–3 year phase-in (five for rural)?

On “staff up during the 2–3 year phase-in,” we got traction by creating weekend-option RN roles with a 15% diff and an internal float pool, plus 6-hour overlap NA shifts to boost HPRD during meals/med pass without full FTEs. Small caveat: make sure your timekeeping splits direct-care vs in-service hours — we got flagged until we recoded. Anyone using tele-nurse triage overnight to support the onsite RN while hiring?
